You must submit a Claim Form in order to receive credit monitoring, CCPA Payments, Alternative Cash Payments and compensation for lost time and out-of-pocket expenses. The deadline to submit a Claim is July 5, 2025.
This Claim Form should be filled out online or submitted by mail if you received a notice from Smith, Gambrell & Russell International, LLP (“SGR”) in or about January 13, 2023, regarding a data security incident that occurred at SGR between July 19 and July 28, 2021 (the “Data Security Incident”), or if you otherwise believe you were affected by the Data Security incident.
You may receive a payment if you properly and timely complete this Claim Form, the settlement is approved, and you are found to be eligible for a payment.
The Notice describes your legal rights and options. Your claim must be submitted online or postmarked by July 5, 2025, to be considered for payment. You can submit your claim for a settlement award in two ways:

To file online, please log in with the Unique ID from the postcard notice you received. After you log in, you will be taken to the correct form to fill out based on your credentials. Please review the form and the information presented to you carefully. Then, complete the required information, certify the form, and submit it. If you do not have a Unique ID, then you will need to submit your Claim Form by mail. Documents that are uploaded must be clear, readable copies and must be less than 20 MB per file in one of the following formats: jpg, jpeg, png, gif, tif, tiff, doc, docx, xls, xlsx, pdf, txt, rtf, or zip. If you are unable to upload a copy of your supporting documentation as part of the online Claim Form, you will need to mail a printed Claim Form along with your documentation to the Settlement Administrator.
